Genetic testing instructions

1. In your booking email confirmation there is a video link, please watch
2. Please read the instructions and follow how to take your swab test. Ensure you haven't just eaten!
3. Sign both your consent forms
4. Photograph the consent form and email to info@rootcauseclinical.com
5. Put one completed consent form into the white Fagron bag
6. Keep the other one!
7. On the swab tube write your name and date, put the bar code label around the bottom of your tube
8. Put the swab tube into the clear bag, then put this into the white bag THEN put into the white Fagron bag
9. Find a postbox and then get ready for your consultation!
